Leave of Absence
There are many circumstances that would lead to an employee needing a leave of absence. Please see some common scenarios below.
A RELEASE TO RETURN TO WORK NOTICE WILL BE REQUIRED for an employee's own medical leave of absence as well as out-patient day procedures.
**Granite Falls Release to Return to Work FormPlease review your Collective Bargaining Agreement language pertaining to leave of absence for leave types available and any specific leave type details. (GFEA - Section 6.1; PSE - Section 9.1)
It is possible that some of these scenarios and/or benefits could overlap. We are here to help you find the best leave options, even if it's a one-day outpatient surgery. Please call or email to schedule a time to discuss with HR.
Medical leave of absence - An employee that needs to be away from work for medical reason(s).
Medical leave of absence consists of absences greater than (5) five days for a medical reason(s).
**Granite Falls Leave of Absence request form, Granite Falls Employee Medical Need form, and Granite Falls Return to Work Form, required.Family Member Medical Leave of absence - An employee that needs to be away from work for a family members medical needs.
Family member(s) experiencing a medical crisis which requires a Granite Falls School District employee to be away from work intermittently or for more than (5) five days.
**Granite Falls Leave of Absence request form and Granite Falls Family Member Medical Need form required.Maternity, Paternity, Bonding Leave - An employee(s) that needs to be away for maternity/paternity/bonding needs.
Maternity/Paternity/Bonding leave for maternity disability, paternity, foster, and bonding time.
**Granite Falls Leave of Absence request form AND Either Granite Falls Medical Need form for maternal parent or Granite Falls Family Member Medical Need form for paternal parent, or Granite Falls Family Member Medical Need form for bonding/foster care.Family Illness Leave - An employee that needs to be away for a short period at-a-time to travel with, care for, or attend to a family member for medical reasons.
Family illness leave is used to travel with, care for, or attend to a family member experiencing medical care on a short-term basis. (This leave must meet FMLA guidelines)
**Absence entered into Frontline using Family Illness leave reason code. The reasons for time off must fall under FMLA guidelines.Bereavement Leave - An employee that needs to be away on bereavement leave for the loss of a close relative or family member.
Bereavement leave is used to attend funeral or memorial services, participate in executor or estate duties, or grieve.
**Absence entered into Frontline using the Bereavement leave reason code. Please consult CBA for the duration of time provided for each type of occurrence.Personal Leave without Pay - An employee desires to be away from work when it exceeds personal leave balances.
Personal leave time that exceeds any personal leave balance - This is considered leave without pay and requires a leave request form submitted to the superintendent in advance of the leave.
