The student Housing Questionnaire is used to help identify students who are eligible for support through the Kids In Transition (KIT) Program.  If you think you may qualify, print using the link below, and return to the main office of your student's school. Feel free to also contact Cassidy Fouts at 360-283-4506 to fill it out over the phone. The Student Support Advocates (SSA) are also able to help with this process and are at each school.
